Book your appointment for your transaction at the Regional Office. You (or your authorized representative) must personally transact at the Regional Office on your scheduled date.

Only the request for certification and/or authentication of eligibility (or eligibilities) of ONE ELIGIBLE will be accepted per schedule. 

NOTE: For first time request for Certification of Eligibility, the eligible is required to claim the Certificate personally.

Request for Certification/Authentication of March 3, 2024 CSE-PPT Passers

Book your appointment for your Issuance of Certificate and/or Authentication of Eligibility to be submitted to the CSC Field Office in Region 5 nearest your location except CSC Albay FO.  

You (or your authorized representative) must personally submit your document on your scheduled date.

Submit Request for Issuance of Certification and/or Authentication of Eligibility at the CSC Field Office (Except Albay FO)

Submit your Application for Grant of SPELS (Honor Graduate, Barangay Official, Sanggunian Member, etc., EXCEPT MC11) to the CSC Field Office nearest your location except CSC Albay FO. 

Note: You may already include your requirements for Issuance of Certificate and/or Authentication of Eligibility.

Submit Application for Grant of Eligibilities under Special Laws and CSC Issuances (SPELS) at the CSC Field Office

Submit your Application for Grant of SPELS (Honor Graduate, Barangay Official, Sanggunian Member, etc., EXCEPT MC11) at the CSC Regional Office 5. Note: You may already include your requirements for Issuance of Certificate and/or Authentication of Eligibility.
